📅 Save the Date: Sports Physicals & Immunizations

Families, please save the date for this upcoming opportunity!

Yakima Valley Farm Workers Clinic will be offering sports physicals and immunizations at Toppenish Middle School on August 4, 5, and 6 from 8:00 a.m. to 4:00 p.m.

This event is a great opportunity for:
✅ Students who plan to play a sport next school year and need a sports physical
✅ Students who need to receive their required immunizations before the first day of school. Check Immunization Requirements for your student(s) online:

Sports physicals are $10, and immunizations are available with a $5 administration fee. Appointments are required. Please call 509-864-1428 to schedule.

Parent consent and health history forms are required and will be provided on arrival. Families are asked to arrive 15 minutes before their appointment.
